Membership Application

2. Read the membership agreement.
3. Submit the form. 
4. Make payment with cash, check or credit card/ Visa or Mastercard add 3.5% processing fee.                                                                                                                       

Once submitted a response page summarizing your form entries, membership agreement and the details regarding payment options will be provided.  If you have any questions or problems completing this form call us we can take your information over the phone or mail you an application whatever works.  Call 503-324-7819 or 503-970-4639 and ask for Debi or David.

Membership Agreement by submitting this form;

• I acknowledge that Dairy Creek Farm is a diverse farming operation and will be selling produce to other outlets in addition to the CSA members. 
• I will share in the bounty and assume the risks of the harvest with the farmer. 
• I will make arrangements to collect my share each week.
• I understand that there are no refunds if I do not collect my weekly share. 
• I am allowed to make prior arrangements to miss up to two of my weekly shares and cash in the credit at the farm by picking equal poundage of canning or storage vegetables.
